Help anyone? Would like to port my OS, apps and files from old PC to new.

You can tell I'm inepxerienced in such things by the very questions I guess. Is there an easy way to transfer one's system from an old PC to a new one? Is it just a question of hard drive data, and if so, what's the *easy* way of doing this?
